What We Do
Primary Care
Charlotte Community Health Clinic
enables a low-income, uninsured
patient the first point of entry into the
health care system and is the
continuing focal point for all his/her
health care services.
Women’s Clinics
Volunteers Physicians and Nurses
provide Women’s Clinics twice a month
(1 morning session and one evening
session) which include Paps Smears,
Clinical Breast Exams, Breast Health
Education, Mammograms
With community support the clinic now offers health promotion, disease
prevention, health maintenance, counseling, patient education, diagnosis and
treatment of acute and chronic illnesses.

Chronic Disease Clinics
Chronic diseases are the major cause of illness, disability and death in the United
States. CCHC joined the NC Chronic Disease Collaborative in 2006 and has a
“state of the art” tracking systems that enables the physicians and patient
information for the continuity of care for diabetes, cardiovascular diseases and
other chronic conditions that need intense medical management.
Health Education
Health Education classes
are provided by both
volunteers and our Health
Educator and Diabetic RN
who conduct classes and  
provide ‘hands on
instructions” allowing our
patients to self-manage
their disease process and
to understand how the
choices they make impact
their health.
Behavioral Health
In Partnership with University of North Carolina at Charlotte, CCHC initiated a
PhD. Behavioral Health Internship. A UNC Charlotte PhD. Student under the
supervision of a PhD. Psychologist provides assessments, intervention
counseling, individual and group sessions and information and referrals.

physicians with ongoing/updated
patient information enabling the
continuity of care for diabetes,
cardiovascular diseases and other
chronic conditions needing intense
medical management.  Patients
receive glucometers and strips,
nutrition and diet education and
educational programs for foot care,
eye care and smoking cessation.
